The Referral Book was created with the purpose of transforming the way professionals, agencies, and individuals access and navigate community resources. By increasing transparency and improving visibility of available services, we strive to make it easier for individuals to connect with essential resources and better understand the support options available to meet their unique needs.
Designed with a focus on serving Medi-Cal members, uninsured and underinsured individuals, and communities facing financial barriers to care, The Referral Book highlights resources that may be accessible to people facing financial barriers. We recognize that navigating available support can be challenging, and our goal is to simplify the process by helping professionals and individuals find and compare community resources.
Our approach differs from traditional resource navigation models by focusing not only on increasing community awareness of available services, but also on strengthening collaboration among agencies throughout the county. Through improved access to information and enhanced partnerships, The Referral Book aims to create a more connected, informed, and responsive network of support for the communities we serve.
How Resource Listings Are Added
Resource listings are either submitted directly by organizations or added by The Referral Book using publicly available information. Organizations are encouraged to review their listings and provide updates to help maintain accurate and current information within the directory.
Because programs, services, eligibility requirements, hours, and availability may change over time, users are encouraged to confirm information directly with the organization before accessing or referring someone to a service.
